Midlands Hub Training Exchange - Participant booking form

Please complete the following booking form once you have selected a Midlands Hub Training Exchange course on offer from your university.  The completed booking form will be sent to the offering university, who will confirm whether or not you have a place.  Please note, spaces on these courses are limited and are allocated on a first come first served basis.

Please also note that bookings on Midlands Hub Training Exchange courses close one week before the course is due to take place.
