Contact the disability advisers at the institution as soon as you have been accepted. You need to start the process at the earliest opportunity. 

If you are funded by a research council, get in touch with the disability advisers at the institution you are attending as soon as you are awarded your bursary/studentship.

You may need a new assessment of your requirements, carried out by an accredited accessment centre. The difference between research degree study and undergraduate or taught masters programmes means that your support requirements may also be different, e.g. you may be expected to attend conferences, to give presentations, have the opportunity to teach undergraduates. A research degree has  different demands  and it is good to look at the implications of this  at an early stage

Alongside this you will need to consider whether to disclose your disability.
